What is Life Stage Transition support?
Life stage transition support helps you through major changes – finishing school, starting work, moving out of home, changing living arrangements or adjusting after a change in health. These moments can feel overwhelming, especially with a disability.
We help you plan ahead, build the skills and connections you need, and settle into new routines with confidence.
More Than Just Assistance
We walk beside you through each stage, so change feels less daunting and more like a fresh start.
- Transition planning and goal setting
- Support moving from school to work, study or day programs
- Help moving out of home or into a new living arrangement
- Adjusting to new routines, places and people
- Connecting with services, supports and community networks
Transition Support That Fits Your Everyday Life
Every transition is different. We take time to understand where you are now, where you want to be, and what support will help you get there.
We work closely with your family, support coordinator and other providers so everyone is on the same page.
Not Sure Where to Start?
Finding the right personal care support can feel overwhelming at first. Our team is trained to walk you through your options, explain your choices clearly, and help you get started with the right personal care support - with zero fuss.
